What to expect on your first visit to our office:
The doctor will review your completed registration packet to learn about you, your health history and your health care goals. A thorough exam will then be conducted to locate the cause of your health concerns. The doctor will discuss the findings and let you know whether you are a good candidate for chiropractic care and if radiographic pictures are necessary.
What should you bring to your first visit:
- Completed registration forms
- Insurance card
- Recent X-rays (In the last year)
If you are seeking care for an automobile collision you also need to bring:
- Completed registration forms
- Claim number and claim representitive information.
- Vehicle collision packet, in addition to the patient registration packet.
If you are seeking care for a work related injury please bring:
- Completed registration forms
- Labor and Industries or Self-insured claim information.
